For anyone paid through this Department, the following documentation must be completed by both staff and students.
- Personnel Information Form
- Payroll Bank Authorization Form
- TD1 Federal Personal Tax Credits Return Form
- TD1 Ontario Personal Tax Credits Return Form
Should there be a change of address or banking information, this must be updated immediately to avoid misdirected pay stubs and/or payments. NOTE: Students updating a change of address through ROSI must also update this information through HRIS (the Payroll System). These two systems are not linked.
